Packages for your care

We receive Government funds to provide care services to older people in South-Eastern Sydney through two programs:

CHSP

Suitable for people with basic care needs who live a fairly active life and need only occasional help. We offer help with housework, shopping, respite, centre based day care, attending a social activity or transport and individual social support.

HCP

Suitable for people who are less active and have medium- to high-level care needs but want to remain living at home. Tailored packages can include personal care, housework, nursing care, transport, shopping, individual support, respite etc. We provide all four levels of care under the HCP

Services are subsidised by the Australian Government Department of Health. But consumers may be asked to contribute towards the cost of their services.

Are you eligible?

You may be eligible for government funding if you are an older person (usually 65+ or 50+ for Aboriginal and Torres Strait Islander people) finding it harder to do the things you used to do. You will need to have a face-to-face assessment of your care needs through My Aged Care to find out if you are eligible.

We can assist you to navigate the aged care system. Call our Intake Officers on (02) 9136 4788, and speak to our bilingual staff about your needs (TIS Interpreting Service is available if we don’t speak your language). For more information drop into our Rockdale office or contact My Aged Care.
