Part-time – 21 to 28 hours per week; Fixed-term until 30 June 2020

We are currently seeking to employ two motivated and passionate persons with a commitment to social justice to join our Help@Home team.

The Case Manager will support and work with frail aged with their specific needs and goals to maximize their choices and independence. The Case Manager is responsible for alignment quality care plans with client specific needs and effective service delivery through Consumer Directed Care Model (CDC) within a person-centred, wellness & enablement approach and client satisfaction.

Essential Criteria:

  1. Fluent in English and specific community language
  2. Tertiary qualifications in social work, aged care, allied health  or demonstrated competence/ experience
  3. Proven knowledge of consumer-directed care and understanding of community aged care services and its requirements.
  4. Demonstrated experience in assessment, case management and reporting within the community aged care sector
  5. Excellent communication skills both written and verbal
  6. Ability to respond to crisis situations, good negotiation and conflict resolution skills
  7. Sound computer literacy including CRM
  8. Demonstrated ability to work with all clients, carers, service providers and others in the community in the community and ability to act with integrity and maintain confidentiality all the times
  9. Demonstrated ability to work independently as well as an effective member of the highly interacted aged service team;
  10. Demonstrated Commitment to ADS values and philosophy and person centre practice
  11. Current driver’s license and a reliable vehicle with comprehensive insurance for work and ability to travel regularly

Desirable Criteria:

  1. Demonstrated skill  in CALD community engagement
  2. First Aid certificate


  • Social, Community, Home Care and Disability Service Industry (SCHCADS) Award, NSW and salary packaging.
